First-time Patients:

To ensure a seamless, efficient, and personalized experience, we kindly ask new patients to observe the following:

• All appointments must be confirmed with a credit card. 
• A cancellation fee will be applied for cancellations less than 24 hours in advance or no shows. 
• Click the links download the New Patient and Credit Card Authorization forms. 

Payment & Insurance Policies:

• Payment is due at the time of service.
• We accept most major credit cards and traveler’s cheques.
• Personal and bank checks are not accepted.
• While we are not affiliated with any insurance plans, we are happy to provide a comprehensive, itemized superbill for submission to your out-of-network benefits provider.
• A follow-up visit within one week for the same illness is included at no additional charge.
